New Patient Registration

registration1Our practice welcomes new patients who live within our practice area. For details of the practice area please see below. In Wales, you can only be a registered patient with one GP surgery, therefore, if you register with us you will be de-registered with your current GP surgery and your previous medical notes will be sent to us. Please call in at the surgery to complete the appropriate forms with a receptionist. Practice Area

Please see the practice boundary map below to determine whether you live in our practice area. Our practice is, as has been the case for a couple of decades, at full capacity, leading to our practice practice boundary, contractually agreed with Betsi Cadwaladr University Health Board, being strictly adhered to. This gives us a degree of control over our increasing list size (currently around 12,000 patients), helping us continue in our aim of providing the highest quality of care. A small proportion of our registered patients live outside our practice boundary, and we appreciate that this has sometimes led to uncertainty in some quarters about our practice boundary; however, this reflects our historical moving out of practice area rules, not our present day ones. Our present day rules mean that if you are already registered with us, and move to live outside our practice boundary, you will be asked to register with a GP practice that has a practice area covering your new address. In recent months we have taken calls from patients living outside our practice boundary, often just a few streets away, asking whether they would be able to register with us; but, for the reasons outlined above, we are unable to make any exceptions. If you still require clarification about our practice boundary, please telephone our practice on 01492 515787 and ask to speak to Tracey Williams. Thank you.
